Inter-rater reliability of clinical movement-control tests of the hip for runners with or without knee pain

Abstract [en]

Background:  Long-distance runners are prone to develop imbalance in the musculature of the hip and weakness in the hip abductors and external rotators, which might lead to overuse injuries in the knee. Poor motor control and change in recruitment of the hip musculature can be contributing factors for the movement dysfunctions. To identify uncontrolled movement (UCM) dysfunctions, Comerford and Mottram developed dissociation tests for several joints, i.e. Kinetic Control (KC) test. To our knowledge, no previous studies regarding the reliability of KC tests for the hip have been published. Therefore the aim of the study was to determine the inter-rater reliability of three clinical movement control tests, for the hip on both legs, in runners with or without knee pain. Methods: 36 healthy individuals, running at least once a week for six months, with or without knee pain was included in the study sample. Two independent raters assessed three KC tests: Single leg stance lateral pelvic shift test, Single leg small knee bend test and Side lying top leg turnout lift test on the same occasion, using a standardised test protocol. The inter-rater reliability was analysed using Cohen´s kappa coefficient (κ), 95% confidence interval, bias index, prevalence index and percentage of agreement. Results:  The inter-rater agreement of Single leg stance lateral pelvic shift test was fair (right κ = 0.29, left κ = 0.36); Single leg small knee bend test substantial to almost perfect (right κ = 0.66, left κ = 0.83) and Side lying top leg turnout lift test fair to moderate (right κ = 0.49, left κ = 0.26). Conclusions: This inter-rater reliability study shows acceptable inter-rater reliability for one out of three evaluated tests, the Single leg small knee bend test. The settings and the rating criteria of the Single leg stance lateral pelvic shift test and the Side lying top leg turnout lift test can be discussed and developed further in order to make the tests more reliable
