Abstrakt

Syfte: Att undersöka eventuella skillnader i demografiska (t. ex. kön, ålder) och sjukdomsspecifika variabler (t. ex. smärta, hälsorelaterad livskvalitet) mellan personer som sökt traditionell respektive digital artrosvård i Sverige samt analysera sannolikheten för deltagande i digital vård utifrån dessa variabler. 

Metod: En retrospektiv icke-experimentell tvärsnittsstudie baserad på registerdata från Svenska Artrosregistret (SOAR). Univariabel analys utfördes för att jämföra skillnader i variablerna mellan de som sökt digital och traditionell artrosvård. Multivariabel logistisk regression genomfördes för att bedöma sannolikheten för deltagande i digital vård. Separata analyser gjordes för personer med knä- respektive höftartros.

Resultat: Totalt ingick 156 625 patienter som erhållit traditionell vård och 33 356 deltagare som fått digital vård i studien (knäartros n = 123 675, höftartros n = 67 267). Universitetsutbildning jämfört med enbart grundskola visades sig vara den starkast bidragande faktorn till ökad sannolikhet för deltagande i digital vård (knä: 295 %; Höft 218%). Mindre gångsvårigheter (knä: 72 %; Höft 47%) samt lägre önskan om operation (knä: 53 %; Höft 82%) ökade också sannolikheten för digital vård.

Slutsats: De som söker digital vård är i större utsträckning kvinnor med högre utbildning som upplever en lägre sjukdomsbörda och är mer fysiskt aktiva. Resultaten från denna studie indikerar att de som söker digital vård söker tidigare i sin sjukdom vilket är fördelaktigt för sjukdomsförloppet.

Abstract [en]

Abstract

Purpose: To explore differences in demographic (eg. sex, age) and disease related patient-reported outcome measures (eg pain, health related quality of life) between individuals seeking traditional and digital osteoarthritis (OA) care in Sweden. 

Design: A retrospective non-experimental cross-sectional study based on data from the Swedish Osteoarthritis Registry (SOAR). Univariable analysis was performed to measure differences in variables between patients seeking traditional and digital care. Multivariable logistic regression models where then performed to assess the different variables effect on probability for participating in digital care. The results for knee and hip where separated.

Results: A total of 156 625 patients receiving traditional and 33 356 receiving digital care were included in the study (Knee n = 123 675, hip n = 67 267). Having a university education compared with only an elementary school education was the factor with highest increase in the probability of participation in digital care (Knee: 295 %; Hip 218%). Other variables such as less walking difficulties (knee: 72 %; Hip 47%) and less desire for surgery (knee: 53 %; Hip 82%) also increased the probability for digital care. 

Conclusion: Participants seeking digital osteoarthritis care seem to be females with a higher education, a lesser disease burden and to be more physically active compared to those participating in traditional OA care. The results from this study indicate the digital participants may seek care earlier in their disease with less symptoms which is beneficial regarding the course of the disease.
