    Study DesignRandomized controlled trial. BackgroundLow back pain is a common disorder. Patients with low back pain frequently have aberrant and pain-provocative movement patterns that often are addressed with motor control exercises. ObjectiveTo compare the effects of low-load motor control (LMC) exercise and those of a high-load lifting (HLL) exercise. MethodsSeventy participants with recurrent low back pain, who were diagnosed with nociceptive mechanical pain as their dominating pain pattern, were randomized to either LMC or HLL exercise treatments. Participants were offered 12 treatment sessions over an 8-week period. All participants were also provided with education regarding pain mechanisms. MethodsParticipants were assessed prior to and following treatment. The primary outcome measures were activity (the Patient-Specific Functional Scale) and average pain intensity over the last 7 days (visual analog scale). The secondary outcome measure was a physical performance test battery that included 1 strength, 3 endurance, and 7 movement control tests for the lumbopelvic region. ResultsBoth interventions resulted in significant within-group improvements in pain intensity, strength, and endurance. The LMC group showed significantly greater improvement on the Patient-Specific Functional Scale (4.2 points) compared with the HLL group (2.5 points) (P<.001). There were no significant between-group differences in pain intensity (P = .505), strength, and 1 of the 3 endurance tests. However, the LMC group showed an increase (from 2.9 to 5.9) on the movement control test subscale, whereas the HLL group showed no change (from 3.9 to 3.1) (P<.001). ConclusionAn LMC intervention may result in superior outcomes in activity, movement control, and muscle endurance compared to an HLL intervention, but not in pain intensity, strength, or endurance.     Purpose: The specific objectives were to: 1) describe the level of pain intensity, disability, activity limitation, kinesiophobia and physical capacity in patients with predominantly peripherally mediated mechanical back pain, and 2) investigate whether differences between these patients in physical and psychosocial factors can be distinguished when the patients are further sub-grouped.Relevance: To improve assessment among patients with chronic low back pain (CLBP) it is important to investigate the prevalence of physical and psychosocial features in homogenous sub-groups.Participants: Seventy-one patients with CLBP, 20-60 years old, with peripherally mediated mechanical pain at the the moment for the study, were included and each patient was sub-classified into one of five sub-groups based on their pain behaviour and functional movement pattern (flexion n=20, flexion/lateral shift, n=11, active extension n=23 , passive extension n=8, and multidirectional pattern n=9).Methods: Data on pain intensity (Visual Analogue Scale), disability (the Roland and Morris Questionnaire), activity limitation (the Patient Specific Functional Scale), kinesiophobia (the Tampa Scale of Kinesiophobia) and physical capacity (lifting capacity and trunk extensor endurance) was collected.Analysis: Mean and standard deviation for parametric and median and interquartile range for non-parametric data were used for descriptive statistics. One-way ANOVA for normally distributed data and Kruskal-Wallis for non-normally distributed data were used for analyses of differences between the sub-groups. The subjects were also divided into two age-groups (20-40 and 41-60 years) and measures of physical capacity were compared to women and men at the ages of 34 and 50, respectively, in the general Swedish population using one sample T-test.Results: The patients reported low to moderate pain intensity (3.1/10±2.4), disability (RMDQ (7.27/24 ±4.2) and kinesiophobia (33.4/68 ±7) and these levels were lower than reported levels in other studies including more heterogenous groups of patients with CLBP. The patiens reported activity limitations (PSFS 13/30±23). Lifting capacity and trunk extensor endurance were significantly lower than in the general population in the youngest age-group. No significant differences in pain intensity, disability, kinesiophobia or physical capacity were found between the sub-groups.Conclusions: This research highlights that patients with predominantly peripherally mediated mechanical back pain may differ from other sub-groups of patients with CLBP (e.g., patients with central sensitization as dominating pain mechanism) regarding physical and psychosocial factors. The individual variation in pain intensity, disability, kinesiophbia and physical capacity among the patients indicates the importance to assess these factors in every patient. Due to the fact that there were few patients in the sub-groups, further research is necessary to explore whether there are differences, that we were not able to disingjish, between patients with different movement patterns.

    Bakgrund: Träning under en graviditet är viktigt både ur ett fysiologiskt samt psykologiskt perspektiv. Bristande fysisk aktivitet är en stor riskfaktor för hälsan. Livshändelser såsom graviditet och förlossning kan utgöra en större risk för bristande fysisk aktivitet hos kvinnor. Gravida kvinnor rekommenderas att vara fysiskt aktiva minst 30 minuter per dag. Informationen kring fysisk aktivitet och träning är ofta bristfällig inom mödrahälsovården varför det efterlyses ett närmare samarbete mellan till exempel barnmorskor och fysioterapeuter. Genom att förbättra kvantiteten och kvaliteten på informationen angående fysisk aktivitet hos gravida finns det en möjlighet att korrigera felaktiga uppfattningar och på så sätt främja en god hälsa. Syfte: Syftet var att beskriva kvinnors egen uppfattning om fysisk aktivitet och träning under och efter en graviditet samt upplevelse av mottagen information från sjukvården angående ämnet. Metod: En kvalitativ intervjustudie genomfördes med fem kvinnor, vilka rekryterades via ett strategiskt urval. Därefter genomfördes en innehållsanalys av intervjuerna. Resultat: Analysen av intervjutexterna resulterade i en huvudkategori “Den gravida kroppen - individens eller sjukvårdens ansvar?”. Huvudkategorin innehöll följande kategorier “Kunskapsinhämtning - individens ansvar” samt “Aktivitetsnivå - individens val” med tillhörande underkategorier. Konklusion: Studien visade positiva upplevelser och uppfattningar av fysisk aktivitet och träning i samband med graviditeten. De intervjuade kvinnorna upplevde bristande information från sjukvården gällande detta ämne. De råd som kvinnorna hade fått från sjukvården upplevdes som osäkra och väldigt generella, därför fanns en önskan om att individanpassa råden.

    Inom ramen för denna artikel diskuterar vi såväl den psykosociala som den fysiska miljön i skolan. Vidare argumenterar vi för att skolans miljö är av betydelse för elevernas möjligheter att lära sig, likväl som för deras upplevelser av lärandesituationerna. Särskilt intressant blir då att lyssna till eleverna. Syftet med denna artikel är därför att synliggöra och diskutera elevers röster i förhållande till hur skolan kan bli den bästa platsen för lärande. Detta görs med utgångspunkt i forskningsprojektet ”Skolan suger” eller?, där drygt 200 elever i åldrarna 11–15 år deltog. Eleverna fick skriftligen reflektera över hur skolan ska bli en riktigt bra lärandemiljö. För att utveckla skolan till den bästa platsen för lärande uttryckte eleverna en önskan att kunna påverka sin lärandemiljö, att uppnå ömsesidighet och samspel, att kunna hantera tidsregleringen i skolan, samt att uppfylla behov för välbefinnande.

    "Dammed taxi cab!" A twelve year old student wrote these words in the margins of a questionnaire in school concerning psychosocial well-being. Within this paper they will serve as a point of departure for the discussion. Can the messages found on the sides of the squares intended for an X in a questionnaire be considered non-messages? Or are these expressions indeed messages that are made silent and therefore can be considered silent messages? Then one might wonder what the meanings of the silent messages are that often occurs in questionnaires? Can, or maybe should, we take notice of these silent messages? According to Polanyi every human being has silent and unexpressed dimensions within themselves, which acknowledge situations where we recognise that we know more than we can explain. This is also stressed by Merleau-Ponty, who argued that something exists beyond what is said, and Bateson emphasised that a non-message is also a message - the silence tells us something. Within this paper we will highlight and discuss the significance of silent messages of communication in general, and especially in questionnaires.

    'Dammed taxi cab' - a 12-year-old boy wrote these words in the margins of a questionnaire, and within this paper they will serve as a point of departure for the discussion of the use of questionnaires as a way to voice children's experiences. The overall aim of this paper is to enable understanding of and discuss the use of questionnaires as a way to voice children's experiences. An analysis in accordance with a life-world approach has provided the lens through which to explore messages that can occur in questionnaires. The key research questions that will be explored in an attempt to answer the overall aim are: (i) how can unanswered questions and notes in the margins of a questionnaire be understood? and (ii) why is a questionnaire not always completed, according to the intention of its creators? To develop an understanding of this we analysed a number of questionnaires with unanswered questions and notes in the margins using a life-world approach. During the analysis two themes emerged: silent messages and messages being silenced. Finally, we discuss the legitimacy of questionnaires in childhood research and possible implications for practice when using this method to give voice to children's experiences.

    Syfte:Syftet med litteraturstudien var att undersöka hur en smartphone kan användas inom rehabilitering efter FHS, i vilken utsträckning smartphonen används. Vilka effekter smartphonen kan ge på i det dagliga livet samt betydelsen en smartphone kan utgöra.

    Metod:För att besvara syftet så utfördes en litteraturstudie som innefattar kvalitativ och kvantitativ forskning baserad på åtta studier. 

    Resultat:Smartphonen har många fördelar som ett kompensatoriskt hjälpmedel samt är betydelsefull för personerna i det dagliga livet på många sätt. Signifikanta effekter kunde uppmätas som ledde till att fler aktiviteter både påbörjas och avslutades i rätt tid, därmed så hanteras många av grundproblemen med förvärvad hjärnskada (FHS), såsom svårigheter med minne, planering, initiering och organisering. Smartphonen föredrogs också av användarna framför andra hjälpmedel. 

    Slutsats:Studien visar att de finns stora fördelar med att implementera smartphonen som en del av rehabiliteringen av förvärvad hjärnskada. Vidare visar studien att smartphonen är den del av människors liv oavsett med eller utan FHS.  Det finns behov av vidare forskning inom ämnet då det finns få studier som belyser smartphonens som kompensatoriskt hjälpmedel i dagsläget.

    Abstract [en]

    Background: After pregnancy and childbirth the female body goes through changes in both body and mind. Inadequate research has been done in the field of physical activity and exercise in postpartum women. Swedish counties has not formed a cohesive plan of antenatal care which creates variations throughout the country. Purpose: The aim for this study was to investigate how women are being informed about physical activity and exercise postpartum by Swedish healthcare. Method: To complete this study a survey was made and via a snowball sampling distributed throughout social media. Participants; 1023 women whom represented each county of Sweden. Inclusion criteria; to have given birth in Sweden the last 5 years and understand the Swedish language. Results: Women get limited oral and written information about physical activity and exercise. The information is mainly distributed by a midwife and women rarely get to meet a physiotherapist postpartum. Half of the women that did meet with a physiotherapist used the information but was in general not satisfied with its content. Conclusion: Postpartum activity and exercise is an individual process that should be exerted in consultation with a physiotherapist and the field of physiotherapy should be utilized to a greater extent in Swedish antenatal care.

    Abstract [en]

    The purpose of this study was to evaluate if verbal encouragement had any influence on hand strength measurements. Twenty subjects participated in the study, all women aged between 20 and 62 years. A standardization of testing conditions and verbal encouragement was designed. Three measurements were conducted during 2 days using T.K.K 5401 Grip D. First there was a trial set, and then Test 1 (without verbal encouragement) and Test 2 (with verbal encouragement) were con-ducted. The results showed a significant improvement regarding the test results from Test 1 to Test 2. The subjects also answered a questionnaire about their feelings towards the two test occasions. A majority of the subjects felt that verbal encouragement had an influence on their performance. In conclusion, it could be said that verbal encouragement both had a subjective and objective influence on a subject's performance. In earlier studies, verbal encouragement has shown to influence different personalities in different ways. Thus, this would be an interesting field of study for future research

    Bakgrund: Det föds mellan 115 000 - 120 000 barn i Sverige varje år, där de flesta förlossningar är vaginala och slutar i någon form av bristning. Bristningarna delas in i fyra grader beroende på deras omfattning. Där grad ett och två innefattar vagina och perineum medan en grad tre och fyra ruptur innefattar sfinktermuskulaturen. Studier har visar att upp till 57% av de kvinnor som råkat ut för en sfinkterruptur lider av någon form av inkontinensproblematik tio år efter skadan. Samtidigt som vissa kvinnor först får problem några år efter att skadan inträffat. Uppföljning efter skada av dessa patienter sker antingen av specialistläkare, barnmorska eller av fysioterapeut. Syfte: Att göra en sammanställning av vilka sjukhus i norra Sverige som har en fysioterapeut inkopplad vid en obstetrik analsfinkterruptur som uppstått i samband med en vaginal förlossning. Metod: En enkät skapades online med fyra frågor om fysioterapeutisk uppföljning och kompetens gällande kvinnor som råkat ut för en sfinkterruptur. Enkäten skickades ut i två omgångar till nio sjukhus i norra Sverige tillsammans med ett informationsbrev. De sjukhusen/lasaretten som inte svarade på enkäten togs kontakt med på telefon för att ställa frågorna. Resultat: Elva procent av sjukhusen i norra Sverige erbjuder individuell information på vårdavdelning, tjugofem procent erbjuder hjälp vid behov, tjugotvå procent har kompetens inom vaginal palpation. Jämfört med en kartläggning gjord i södra Sverige där det låg mellan 80-90%. Konklusion: Den fysioterapeutiska uppföljningen och kompetensen kring dessa kvinnor skiljer sig i norra och södra Sverige.

    Bakgrund: Delaktighet i skolans utbildning är något som alla elever har rätt att ta del av. Strävan ska vara att anpassa skillnader i elevernas förutsättning att tillgodogöra sig utbildningen. Elever med funktionsnedsättningar är påvisat mer inskränkta eller begränsade av delaktighet, vilket visar sig i skolans miljö. Syfte: Syftet med föreliggande studie var att beskriva lärarnas erfarenheter av att arbeta med elever som har funktionsnedsättning, i ämnet idrott och hälsa Metod: studiens design var kvalitativ. Sex intervjuer genomfördes med lärare i grundskolan inom skolämnet idrott och hälsa som samtliga hade erfarenhet av arbeta med elever som har funktionsnedsättning. Det insamlade datamaterialet analyserades med stöd och inspiration av en induktiv innehållsanalys och resulterade i fyra kategorier: ”En tillåtande undervisningsmiljö främjar delaktighet”,” Strategier för en främjad delaktighet”, ”Förstärkt kompetens kan främja elever med funktionsnedsättning till delaktighet”, ”Organisatoriska resurser i arbetet gentemot att främja delaktighet i skolan”. Resultat: Lärarna påvisar hängivelse till att inkludera elever med funktionsnedsättning. De rapporterar samtidigt begränsningar i deras kompetens, kännedom, skolans miljö och verktyg som kan resultera i att elever med funktionsnedsättning frånses behovet av anpassning, eller exkluderas. Konklusion: Lärare inom idrott och hälsa i grundskolan upplever att deras görande och skolan som organisation har ett starkt samband till hur elever med funktionsnedsättning får möjlighet till delaktighet. Studien kan användas i syfte att medvetandegöra att mer stöd i skolan behövs för att samtliga elever ska delta i skolämnet idrott och hälsa.

    Objectives: To test the reliability and validity of the Students' Attitudes Towards Addressing Sexual Health questionnaire (SA-SH), measuring students' attitudes towards addressing sexual health in their future professions.

    Method: A cross-sectional online survey (22 items) were distributed to 186 nursing, occupational therapy and physiotherapy students in Sweden, April 2015. Validity and reliability were tested.

    Results: The construct validity analysis led to three major factors: present feelings of comfortableness, future working environment and fear of negative influence on future patient relations. The construct validity, internal consistency reliability and intrarater reliability showed good results.Conclusion: The SA-SH is valid and reliable.

    The aim of this study was to explore differences and similarities in health care students’ attitudes towards working with and communicating with patients about sexual health issues in their future professions. The aim was also to explore whether the students’ gender, age and future professions were influencing factors and whether there was a change in attitude depending on educational levels, gender, age and future professions. The study also aimed to explore the potential development of those differences and similarities in attitudes between health care students having achieved different levels of education and training in their future professions. A cross-sectional quantitative study was performed with an online survey distributed to nursing, occupational therapy and physiotherapy students. The students believed that they needed increased sexual health education and increased communication skills about sexual health. Gender and future profession are factors that significantly affect the attitudes of the students towards working with sexual health. Nursing and occupational therapy students have a more positive attitude towards addressing sexual health in their future professions than do physiotherapy students. Further research is needed in this field to improve competence in sexual health for all student groups, particularly physiotherapy students. Further research is also needed to explore the significance of gender regarding education in sexual health and attitudes towards working with sexual health.

    An anterior cruciate ligament (ACL) injury affects knee proprioception and sensorimotor control and might contribute to an increased risk of a second ACL injury and secondary knee osteoarthritis. Therefore, there is a growing need for valid, reliable and responsive knee proprioception tests. No previous study has comprehensively reviewed all the relevant psychometric properties (PMPs) of these tests together. The aim of this review protocol is to narrate the steps involved in synthesising the evidence for the PMPs of specific knee proprioception tests among individuals with an ACL injury and knee-healthy controls.

The aim ofthis study was to describe young Indian women's body image. One hundred female university students in India answered the Ben-Tovim Walker Body attitudes questionnaire (BAQ). The result showed that Indian women had a sound body image.

    The municipality should offer good healthcare for those living in managed care facilities. This responsibility encompasses both rehabilitation as well as certain technical aids. The purpose of the study was to survey the work procedures of physical therapists in municipalities. A mail survey with questions regarding organization, work procedures, competence and job satisfaction was sent to 96 randomly selected physical therapists employed by Swedish municipalities. 70 physical therapists responded to the survey, which showed that the organization varies widely among different municipalities. However, a common denominator was a large territory with a large number of caretakers. The work of physical therapists largely consists of advising and educating personnel. They have a broad competence which is necessary and in demand but few decision makers know what physical therapists can contribute. Most physical therapists believe that they have a stimulating and meaningful work but that the resources are too scarce

    Abstract [en]

    The experiences from persons in the third age of lifestyle changes due to obesity are rarely described in research. Interventions regarding lifestyle changes and obesity show weak evidence for long-term effects. Information and communication technology (ICT) may add an important potential in interventions, but readiness among older persons and the actual needs for support must be explored. Therefore the overall aim for the thesis was to explore the prerequisites for sustainable lifestyle changes among older persons with obesity; and how this could be supported by ICT.Both qualitative and quantitative approaches were used. In paper I, 10 participants were individually interviewed about their experiences of lifestyle changes and in a multistage focus group study (paper IV), 6 participants expressed the support needed for sustainable lifestyle changes in relation to obesity. Qualitative content analysis was used. In a randomly selected population survey, 154 participants responded to an enquiry about general and health related ICT usage (paper II). They also rated scenarios for expected psychosocial impact on web based e-health services and mobile health applications (paper III).An integration of the results identified four main findings: The first finding, A never-ending process of vigilance and vulnerability, showed constant attempts of weight loss and pre-occupation on not to fail in an un-supportive environment. The second finding; Need for psychosocial support marked that the process of lifestyle changes was influenced by factors as sensibility to moods, social support and others’ attitudes. It was therefore essential for the persons to achieve emotional balance. The third finding, ICT as a possible support for adaptability, presented that older Swedes expected a positive psychosocial impact by ICT, especially in a future perspective. ICT was most valued in the domain of adaptability. The degree of usage of health related ICT, however, was low, although persons with overweight had more experience of this usage. Finally, the fourth finding; Reconstructions from weight to health expresses the needs of reconstructing focus towards health, rather than being pre-occupied with weight loss. Physical activity that was associated to joy and to have a functioning body, a body that would serve them as before, were motives to lose weight.In conclusion, this thesis illustrates that a lifestyle change due to obesity in third age is lined with vigilance not to fail and requires a great deal of endurance and effort. The potential of ICT to support lifestyle changes is likely to be substantiated by the positive expectations on ICT from a psychosocial impact perspective, especially in views of the future. Findings indicate a need for moving focus from weight towards health and to see opportunities, and that psychosocial support and enjoyable physical activity are important prerequisites for sustainable change.

    Managerial documents for the national school system in Sweden have emphasised taking students’ voices as a starting point in forming education, and several previous studies have indicated the benefits of giving students opportunities to participate in school. This study aimed to explore students’ reflections on what they would do if they were to decide how to make school the best place for learning. A total of 200 students aged 11 to 15 years from four schools (rural and urban) in two municipalities in the northern part of Sweden participated. The empirical data consisted of the students’ written reflections. The findings fall within four themes: (i) influencing educational settings; (ii) striving for reciprocity; (iii) managing time struggles; and (iv) satisfying well-being needs. Tensions between the students’ previous experiences and future visions appeared. The findings can offer direction regarding aspects of the learning environment in school that could be improved.

    The aim of this study was to describe, reflect upon, and create a deeper understanding of aspects relevant for promoting a positive school environment from a student perspective. The data was analyzed by using an inductive phenomenological method and based on written responses from 200 Swedish students from grades 5–9. The results indicated that the students found aspects within, as well as beyond, the classroom relevant for a positive school environment. For instance, outings were considered relevant for building and maintaining friendships and for learning processes. Moreover, the students discussed formal and informal conditions and considered social as well as structural circumstances important for having a good time in school. The relation between learning and well-being was also emphasized by the students.

    Incentives for teaching occupational science may vary from advancementof the discipline to providing philosophical grounding for the profession ofoccupational therapy. Regardless of the aim, the essential content of thesubject matter – occupational science – has been largely unexplored. Inseeking to refine concepts of occupation that emerged from scholarshipin occupational therapy and yet distance the conversation from anapplied perspective, occupational scientists have focused heavily on theform, function and meaning of occupation as well as socioculturalfactors effecting occupational engagement. This focus has largelydivorced the content from the exploration of the embodied experienceof and the human capacity for action. This paper seeks to challenge thatdivision by considering human capacity for action as integral tounderstanding occupation, and knowledge of body structure andfunction as key to understanding human capacity for action. We offer anexample of a course where human capacity for action is foregrounded,enabling students to more clearly understand the relationship betweenoccupation, embodied action, and body structures and functions.

För att besvara syftet valdes en empirisk kvalitativ design som är lämpligt för att fånga en persons egna tankar, känslor, upplevelser, erfarenheter och åsikter och den värld den lever i med personens egna ord. Data insamlades genom intervjuer med personer som har läkemedelsassisterande behandling vid opiatberoende. Insamlad data bestod av sex intervjuer som analyserades utifrån en latent kvalitativ innehållsanalys. Analys av data resulterade i ett övergripande tema; Att leva i livet inte överleva livet och fyra kategorier; En kamp att anpassa sig till något nytt och finna struktur i vardagen. Att våga tro på sin förmåga. Att få professionellt stöd på vägen. Den sociala miljöns betydelse för att kunna bryta med det gamla livet. Resultatet visade att engagemang i vardagliga aktiviteter är en stor utmaning och en kamp som både beror på vilket stöd personerna får av vård och socialtjänst i sin förändringsprocess, deras tilltro till sin egen förmåga och hur det lyckas skapa en ny identitet samt hur den sociala miljön runt dem ser ut.  Slutsatser som kan dras utifrån studiens resultat kan ge implikationer för att personer som har läkemedelsassisterande behandling vid opiatberoende vill ha hjälp med att bygga upp det nya livet utöver att de får mediciner. Det är en kamp att anpassa sig och skapa en ny identitet och i det arbetet kan arbetsterapi spela en viktig roll.

    Introduktion: Runt om i Sverige finns olika idrottslabb som utför fystester. Det finns krav på idrottsrörelsen för att utveckla parasporten och till vår kännedom finns ingen fysprofil publicerad för personer med funktionsnedsättning. Paraidrottare har olika diagnoser och i denna studie modifierades fystester för personer med cerebral pares, spina bifida, trauma och polio. Syfte: Modifiera, anpassa och utvärdera Sveriges Olympiska Kommittés fysprofil för paraidrottare med olika typer av diagnoser där personerna har funktion i övre extremitet, samt sitter i rullstol. Metod: Femton personer deltog i studien efter förfrågan från Svenska Handbollsförbundet. Fystesterna utfördes i tre omgångar på Blekinge Health Arena. I studien ingick även en kvalitativ mätning i form av ett självskattningsformulär. Testerna som utfördes var gripen, dips, chins, bänkpress, bänkdrag, sprint, Harres och Beep-test. Resultat: Chins, bänkpress, Beep-test, Harres och sprint modifierades och anpassades. Resterande tester blev desamma som originaltesterna. Resultaten visade att det var möjligt att utföra de modifierade fystesterna för de olika diagnoserna. Självskattningsformulären visade att deltagarna tyckte att testerna var ett bra sätt för att testa deras idrottsprestation. Konklusion: Det var möjligt att modifiera och anpassa Sveriges Olympiska Kommittés fysprofil för paraidrottare. Modifierade fysprofilen är användbar för paraidrottare och testar de parametrar som är viktiga.

    Abstract [sv]

    Syfte: Studiens syfte var att undersöka hur yrkesarbetande föräldrar med barn i åldrarna 6-10 år upplever sin aktivitetsbalans.

    Metod: Semistrukturerade intervjuer genomfördes med 14 yrkesarbetande föräldrar. Intervjuerna genomfördes med en deltagare åt gången. De transkriberade intervjuerna analyserades med en kvalitativ innehållsanalys vilken slutligen resulterade i fyra kategorier.  

    Resultat: Resultatet visade de fyra kategorierna: “Att få livspusslet att gå ihop” som beskriver de svårigheter deltagarna har att få rätt mängd och rätt variation mellan aktiviteterna. “Stressande arbetssituation” beskriver deltagarnas upplevelse av hur deras aktivitetsbalans påverkas av arbetet. “Egna aktiviteter som kraftkälla” beskriver deltagarnas upplevelse av att få tid till egna aktiviteter. Samt “Rekreation ger återhämtning” som beskriver vad deltagarna upplever som återhämtning och hur det påverkar övriga aktiviteter.

    Slutsats: Slutsatsen är att deltagarna inte upplever aktivitetsbalans i vardagen. Detta beror på att lönearbetet tar en stor del av den vakna tiden och att barnen och deras välbefinnande prioriteras. Deltagarnas upplevelse är att de har balans när vardagen flyter på utan störande moment och aktiviteterna harmonierar samt att vara lugn och ostressad själv.

    Konsumenttester av vinterskor och halkskydd2015Rapport (Övrigt vetenskapligt)
    Abstract [sv]

    Sjukvårdsbaserad skadestatistik från Sverige visar att singelolyckor med fotgängare på is och snö, orsakar höga skadetal. I Sverige beräknas ca 10000 män och 15000 kvinnor uppsöka sjukvård på grund av skada vid fall på snö och is. Personer skadade i fallolyckor på snö och is har svårare skador och längre konvalescenstid jämfört med fotgängare som faller på barmark. Det är därför viktigt att identifiera preventiva metoder och hjälpmedel för fallolyckor vintertid.Syftet med detta arbete har varit att genomföra konsumenttester av vinterskor och halkskydd på olika underlag med avseende på hur väl de fungerar som skydd mot att halka samt hur användarvänliga de upplevs av brukaren. I arbetet har det också varit viktigt att försöka identifiera kriterier som kan anses vara betydelsefulla för att bedöma vinterväglagsegenskaperna. Dessutom fanns ett behov av att ta fram underlag och förslag på en standardiserad metod för halkskydds-/vinterskortester och en märkning som ger konsumenterna möjlighet att bedöma och jämföra halkskyddens respektive vinterskornas egenskaper innan köp.Testmetoden som tagits fram för att undersöka halkskyddens och skornas egenskaper bygger på tidigare erfarenheter från tester av halkskydd. Metoden bygger på att försökspersoner genomför gångtester med halkskydd och skor på olika underlag samtidigt som olika mätmetoder, både objektiva och subjektiva, används för att kategorisera gång. Tester har genomförts av 9 försökspersoner, över 45 år, 4 män och 5 kvinnor, som testat 19 halkskydd och 20 olika skor. Fyra olika underlag har använts för att kategorisera egenskaperna på de undersökta halkskydden och skorna: is, is täckt med snö, packad snö, torr betong.Testmetoden har kompletterats med laboratoriemätningar av halkskyddens och skosulornas friktion på is samt hårdheten hos skornas sulor. Multipel hierarkisk multipel regressionsanalys användes för att identifiera vilka kriterier som kan ha betydelse för fotgängarnas skattade helhetsupplevelse av de testade halkskydden på olika underlag.Resultatet av förmätningarna visar att testgruppen (fem kvinnor och fyra män med medålder 47 år) motsvarar en normalpopulation, avseende variationer i de uppmätta bakgrundsparametrarna.FIOH:s friktionsmätningar på is av samtliga skor och halkskydd visar att halkskydden har genomgående högre friktionsvärden än skorna, dvs. de har bättre fäste på is än de skor som testats men resultaten visar på små skillnader mellan de sämsta halkskydden och de bästa skorna.Resultat från friktionsmätningen, som den utförs idag i fixerat utförande, ger inte överensstämmelse med försökspersonernas upplevelser av gångsäkerhet eller fallrisk. Försökspersonernas gångcykel med hälisättning, överrullning och fotavveckling bygger upp den samlade uppfattningen om egenskaperna och fångas enbart i de subjektiva mätmetoderna.Sex kriterier som bedömts viktiga att ta hänsyn till när det gäller halkfri gång med en vintersko har kunnat identifieras via analys av försökspersonernas fritextsvar avseende fördelar, nackdelar och övrig funktionalitet: gångsäkerhet, möjliggör ett naturligt gångmönster, stabilitet, förutsägbarhet, flexibilitet och passform/komfort. Gångsäkerhet är viktigast och omfattar fäste/grepp, detta är viktigast på is, is/snö vid gång samt vid start och stopp.Sju subjektiva kriterier för halkskydd har också identifierats; gångsäkerhet, tillåter naturligt gångmönster, stabilitet, förutsägbarhet, flexibilitet, passform/komfort, och ljud. Gångsäkerhet är viktigast.Den självskattade hälisättningen på underlagen ren is samt på snö på is framkom som det viktigaste kriteriet för testpersonernas helhetsupplevelse av skyddet. Även skattad fotavveckling på alla underlag med främst på främst på snö på is var av betydelse. Upplevd balans och fallrisk var mest utslagsgivande för helhetsupplevelsen av de halkskydd som inte valdes för eget bruk. Det tyder på att en lägre grad av skattad kontroll av kroppshållning och balans under gång med skyddet är en relevant indikator för att identifiera skydd med bristande funktioner.Variablerna subjektivt skattad fallrisk, balans, hälisättning och fotavveckling kan samtliga relateras till ett skydds halkegenskaper och funktion. Upplevd fallrisk samvarierar med såväl balans, hälisättning och fotavveckling vilket innebär att kriteriet upplevd fallrisk kan antas omfatta de övriga och kan väljas som en enskild variabel i fortsatta analyser.Kvalitetsnivåer i bedömningsskalor kan antas gå att knyta till de subjektiva kriterierna.En standardiserat testmetod för klassindelning av egenskaper för vinterskor respektive halkskydd bör bygga på mätmetoder med försökspersoner enligt ovan.Fortsatta studier med fler testpersoner behövs för att verifiera de resultat som indikeras ovan.

    Abstract [en]

    Kneeblocks are used to create a symmetric sitting position for individuals with different physical disabilities. The aim of this paper was to examine the impact of kneeblocks in children with cerebral palsy. Six children aged 4-13 years (mean 9 years) participated in the study. Pressure applied to the children's legs was measured with a computer aided system with waterfilled plastic bags and a pressure transducer with 2 Hz registration continuously during 3 minutes. The registrations were made in four difference sitting positions - horizontal sea and 5 degrees forward inclined, seat with and without a hipbelt, while the children performed different standardised tasks at a table. This was documented by videofile and photographs. The results were presented for the different sitting positions and tasks. The means pressure varied from 0.5 kPa in the sitting position with horizontal seat with a hipbelt to 5.2kPa in the position with 5 degrees forward inclined without a hipbelt. Occasionally pressure up to 24.5 kPa was measured. The kneeblocks did constantly exert pressure on the children's legs, but the pressure varied. If kneeblocks are used it should be in combination with a hipbelt, to minmise the pressure applied to the children's legs.

    Abstract [en]

    Recent studies have indicated that the deadlift exercise may be effective in decreasing pain intensity and increasing activity for most, but not all, patients with a dominating mechanical low back pain pattern. This study aimed to evaluate which individual factors measured at baseline could predict activity, disability, and pain intensity in patients with mechanical low back pain after an 8-week training period involving the deadlift as a rehabilitative exercise. Thirty-five participants performed deadlift training under the supervision of a physical therapist with powerlifting experience. Measures of pain-related fear of movement, hip and trunk muscle endurance and lumbopelvic movement control were collected at baseline. Measures of activity, disability and pain intensity were collected at baseline and at follow-up. Linear regression analyses were used to create models to predict activity, disability and pain intensity at follow-up. Results showed that participants with less disability, less pain intensity and higher performance on the Biering-Sørensen test, which tests the endurance of hip and back extensor muscles, at baseline benefit from deadlift training. The Biering-Sørensen test was the strongest predictor since it was included in all predictive models. Pain intensity was the next best predictor as it was included in two predictive models. Thus, for strength and conditioning professionals who use the deadlift as a rehabilitative exercise for individuals with mechanical low back pain, it is important to ensure that clients have sufficient back extensor strength and endurance and a sufficiently low pain intensity level to benefit from training involving the deadlift exercise.

    STUDY DESIGN:Randomized controlled trial.

    OBJECTIVE:

    The aim of this study was to compare the effects of low-load motor control (LMC) exercises and a high-load lifting (HLL) exercise, on lumbar multifidus (LM) thickness on either side of the spine and whether the effects were affected by pain intensity or change in pain intensity.

    SUMMARY OF BACKGROUND DATA:

    There is evidence that patients with low back pain (LBP) may have a decreased size of the LM muscles with an asymmetry between sides in the lower back. It has also been shown that LMC training can affect this asymmetry. It is, however, not known whether a high-load exercise has the same effect.

    METHODS:

    Sixty-five participants diagnosed with nociceptive mechanical LBP were included and randomized into LMC exercises or a HLL exercise, the deadlift. The LM thickness was measured using rehabilitative ultrasound imaging (RUSI), at baseline and after a 2-month training period.

    RESULTS:

    There were no differences between interventions regarding effect on LM muscle thickness. However, the analysis showed a significant effect for asymmetry. The thickness of the LM muscle on the small side increased significantly compared with the large side in both intervention groups, without influence of pain at baseline, or change in pain intensity.

    CONCLUSION:

    At baseline, there was a difference in thickness of the LM muscles between sides. It seems that exercises focusing on spinal alignment may increase the thickness of the LM muscles on the small side, irrespective of exercise load. The increase in LM thickness does not appear to be mediated by either current pain intensity or the magnitude of change in pain intensity.

    Background Context

    Assessment of posture and lumbo-pelvic alignment is often a main focus in the classification and treatment of patients with low back pain. However, little is known regarding the effects of motor control interventions on objective measures of lumbo-pelvic alignment.

    Purpose

    The primary aim of this study was to describe the variation of sagittal lumbo-pelvic alignment in patients with nociceptive mechanical low back pain. The secondary aim was to compare the effects of a high-load lifting exercise (HLL) and low-load motor control exercises (LMC) on change in lumbo-pelvic alignment with a special emphasis on patients with high and low degrees of lumbar lordosis (lu) and sacral angle (sa).

    Study Design

    This study is a secondary analysis of a randomized controlled trial evaluating the effects of HLL and LMC.

    Patient Sample

    Patients from the primary study, i.e. patients categorized with nociceptive mechanical low back pain, who agreed to participate in the radiographic examination were included (n=66).

    Outcome measures

    Lateral plain radiographic images were used to evaluate lumbo-pelvic alignment regarding lumbar lordosis and sacral angle as outcomes with posterior bend as an explanatory variable.

    Methods

    The participants were recruited to the study from two occupational health care facilities. They were randomized to either the HLL or LMC intervention group and offered 12 supervised exercise sessions. Outcome measures were collected at baseline and at the end of intervention period 2 months after baseline.

    Between and within group analyses of intervention groups and sub-groups based on the distribution of the baseline values for lumbar lordosis and sacral angle respectively, (LOW, MID, HIGH), were performed using both parametric and non-parametric statistics.

    This study was supported by two grants from Visare Norr (12000 US dollars) and Norrbotten County Council (13000 US dollars). The supporting organizations were not involved in the collection of data, analysis, interpretation or drafting of the manuscript.

    Results

    The ranges of values for the present sample for lumbar lordosis were 26.9-91.6° (M=59.0°, SD=11.5°) and for sacral angle were 18.2-72.1° (M=42.0°, SD=9.6°). There were no significant differences between the intervention groups in the percent change of either outcome measure. Neither did any outcome change significantly over time within the intervention groups. In the sub-groups, based on the distribution of respective baseline values, LOWlu showed significantly increased lumbar lordosis, whereas HIGHsa, showed significantly decreased sacral angle following intervention.

    Conclusions

    This study describes the wide distribution of values for lumbo-pelvic alignment for patients with nociceptive mechanical low back pain. Further research is needed to investigate sub-groups of other types of low back pain and contrast findings to those presented in this study. Our results also suggest that re-training of lumbo-pelvic alignment could be possible for patients with low back pain.

    Abstract [en]

    Purpose: The aim of this study was to explore variables influencing success or failure of eight weeks of high load motor control training with the conventional deadlift exercise.Relevance: Researching viable exercises for rehabilitation of specific sub-groups of persistent low back pain is relevant for physical therapists in order to develop tailored treatment regimes for patients with persistent low back pain. This study contributes to this research by exploring which variables characterize the ideal patient for the conventional deadlift exercise.Participants: Thirty-five patients with persistent mechanical low back pain were recruited consecutively from two occupational health care services in Umeå, Sweden . Inclusion and exclusion criteria were designed to include patients with persistent mechanical low back pain.Methods: The study design was a prospective cohort study. The intervention consisted of eight weeks of training with the conventional deadlift exercise. To discriminate between patients with a successful or failed outcome of treatment, change in the patient-specific functional scale was used and a cut-off at 50 % improvement was set. Possible predictive variables collected at baseline included physical activity level, pain intensity (Visual Analogue Scale), activity limitation (the Roland and Morris Disability Questionnaire and the Patient-Specific Functional Scale), kinesiophobia (Tampa Scale of Kinesiophobia), specific anamnestic questions regarding patients' history and symptoms of low back pain, test of active movement control of the low back, trunk muscle endurance(Prone bridge test, Side-bridge test, Biering-Sörensen test) and lift strength (static two-hand lift test), two-point discrimination of the low back and ultrasound imaging of the mm. multifidi.Analysis: Student´s T-test for normally distributed continuous data, Mann Whitney for non-normally distributed continuous data and chi-square tests or Fisher´s Exact tests for categorical variables were used for analyses of differences between the success and the failure group.Results: No significant differences between groups were found in background, anamnestic or physical performance variables. After eight weeks of training, 15 patients (43 %) were categorized as treatment success and 20 patients (57 %) were categorized as treatment failure according to the cut-off set for the PSFS. The patients reported difficulty in performing a wide variation of activities, ranging from not being able to sit for longer than 15 minutes, to stand upright and watch their children play football games, and to not being able to run long distances, play football or perform different lifting tasks.Conclusions: We conclude that the conventional deadlift exercise may be considered a possible exercise to improve patients' activity limitations, if administered by a therapist experienced in resistance training and analyzing movement patterns. However, further research is needed to explore which variables can define patients in the successful and in the failure group, respectively.Implications: The results of this study imply that the conventional deadlift exercise can be used in treatment of patients with mechanical low back pain in order to increase activity limitation. However, it is still unclear on what grounds treatment with the conventional deadlift exercise is indicated to achieve these results.

At the same time the cold climate offers opportunities that can be utilized in an innovative manner, both technically and aesthetically, to develop the winter environment to be attractive and health promoting in outdoor activities.The aim of the study was to develop a methodology for giving voice to citizens to be used in the process of urban planning and design for good health in a cold climate. The qualitative investigations of citizen’s experiences of health promoting aspects included two workshops focused on the following questions; What are your experiences of health and well-being in a cold climate? How can this city be planned to promote health during the winter season? The 53 participants came from two cities in the northern part of Sweden. At Workshop 1 small groups of citizens (5-8) were asked to make a collective collage cutting and pasting images from magazines without talking to each other while soft music was playing in the background.When finished the silence was broken and the participants presented their contributions. At Workshop 2 small groups of citizens (5-8) were given maps of the city with different paths for walking tours aimed to pinpoint areas for improvement. After the walk the group presented their ideas for each other and a number of innovative ideas surfaced.Including citizens in the process of urban planning is not only a good source of ideas and information useful in the planning process but may be an opportunity to strengthen health literacy and healthy behavior. This study may serve as an example of an empowerment - based method including citizens in the process of urban planning and design.

    Abstract [en]

    In this paper we will explore and discuss possibilities to learn and research through an appreciative approach in order to enhance continuing professional development for teachers and leaders in school systems. Traditionally humans are used to define problems and trying to find solutions. This involves identification of the bad or threatening and preventing it from taking place, in other words prevention. What would happen if we choose to solve problems in a different way: identify what we appreciate and let this be the base for continued development and improvement? A promoting perspective and salutogenic focus has shown to be successful in helping humans increase their experience of health, which may also create opportunities for learning. How would it be if we changed our point of departure in learning and research processes from problems to appreciation?Our thoughts about Appreciative Based Learning (ABL) has evolved while working with Problem Based Learning (PBL) as teachers. When we came in contact with Participatory and Appreciative Action Research (PAAR) and Appreciative Inquiry we sensed an increased opportunity for positive change, a new way of working with education and research. PAAR can be seen both as a process of building and sustaining change in organizations (Participation, Appreciation, Action and Reflection) and also as a research methodology. PAAR has its roots in action research which is a problem based method aiming at developing and improving practice. Action research can be seen as participatory and collaborative research. A central question in this kind of research is how change of practice can be done, individually or collectively. The new aspect in PAAR is appreciation. An improvement process starts from for example acknowledging factors that work well in an organization or for an individual and the next step is to amplify these positive factors. PAAR research seeks for example to explore positive experiences of a phenomenon and how these experiences can be amplified and sustained in a practice.Central aspects of ABL are appreciation, participation, multimodalities, learning from each other and reflection. Just like PAAR, ABL can offer an opportunity to take positive questions and turn them into positive actions. We will give examples of how we as teachers and researchers have applied ABL in our own teaching and research. Finally we raise questions of how ABL can play a role in school improvement and educational research, building not only a better school but a better future as well.Methodology or methods/research instruments or sources used:We will focus on how to use ABL in the field of school improvement generally and specifically pertaining to educational research, through giving the theoretical background and examples of how we have applied ABL. We have been inspired by ABL in our own research, when teaching in higher education and when working with continuing professional development for teachers and leaders in school systems.Conclusions or expected outcomes or findings:This paper seeks to introduce and explore ABL as a method in school improvement processes and in educational research. We will share our process in developing ABL; how we have applied ABL in our teaching and research. We hope that our thoughts about the possibility to learn and research through an appreciative approach will inspire the participants to reflect on their own practice.

    Abstract [en]

    The aim of this paper is to describe and discuss Appreciative Inquiry (AI) as a research method to facilitate student voice in school research. The paper sets out a model for conducting AI in schools. The research questions identified are: What are the researcher’s reflections when using Appreciative Inquiry in school research? What challenges and opportunities can be found when using Appreciative Inquiry in research processes with students? An application of the model will be presented and problematised. The conclusions indicate the importance of inviting students to participate in the process of defining research topics, and of using multimodal methods for facilitating students’ exploration of school experiences. It is also vital that adults support students in imagining and articulating visions on how to improve the school, as well as plans designing for enacting visions. Finally, a ‘whole school approach’ is emphasised for contributing to sustainable appreciative student voice work in schools.

    Abstract [en]

    This article focuses on student participation in the research process as a contribution to school improvement. The specific aim of this article was to explore students' participation in different phases of a research process and discuss how their participation can contribute to school improvement. Based on a life-world phenomenological ontology, we used two research and development projects - Full of Value and Arctic Children - to shed light on participation in research. When doing research together with students, we have been inspired by Participatory Appreciative Action Research (PAAR). The methods used in the projects were open writing, group reflection, drawings, and exhibition discussions. This research showed that students were able to explore and express their lived experiences of behaviour and well-being in school, and how this was linked to positive change. We found students trustworthy, capable, and competent, enriching the process of school improvement.
