    Abstract [sv]

    Att som förälder få uppleva att barnet drabbas av en kronisk sjukdom är en omvälvande händelse för många föräldrar. När ett barn blir sjuk är det oftast en hel familj som drabbas och i vården finns det en utmaning i att möta hela familjen som är i behov av stöd och vård. Syfte var att beskriva upplevelser av att vara förälder till barn med kronisk sjukdom. I denna litteraturstudie utgick vi från Graneheim och Lundmans metod för kvalitativ innehållsanalys med en manifest ansats. Resultat visar att diagnosen och sjukdomen upplevdes svår att hantera. Känslor som sorg, förtvivlan och skuld beskrevs komma i samband med sjukdomen. Känslan av att behöva stöd från hälso- och sjukvården upplevdes viktig av föräldrarna. Familjen upplevde svårigheter att hantera det vardagliga livet och få det att fungera. Mycket oro över det sjuka barnets framtid upplevdes. Fortsatt forskning som handlar om hur hälso- och sjukvården ska kunna ta hand om föräldrar till kroniskt sjukt barn bör utföras. Hälso- och sjukvårdspersonal behöver få tillgång till verktyg som hjälper dem att bemöta föräldrar till kroniskt sjuka barn.

    Abstract [en]

    ObjectivesPatients admitted to an intensive care unit (ICU) often experience distressing memories during recovery that have been associated with poor psychological and cognitive outcomes. The aim of this literature review was to synthesise the literature reporting on relationships between elements of ICU treatment and memories after discharge in adult ICU survivors.Review method usedIntegrative review methods were used to systematically search, select, extract, appraise and summarise current knowledge from the available research and identify gaps in the literature.Data sourcesThe following electronic databases were systematically searched: PubMed, Ovid EMBASE, EBSCOhost CINAHL, PsycINFO and Cochrane Central Register of Controlled Trials. Additional studies were identified through searches of bibliographies. Original quantitative research articles written in English that were published in peer-review journals were included.Review methodsData extracted from studies included authors, study aims, population, sample size and characteristics, methods, ICU treatments, ICU memory definitions, data collection strategies and findings. Study quality assessment was based on elements of the Critical Appraisal Skills Programme using the checklists developed for randomised controlled trials and cohort studies.ResultsFourteen articles containing data from 13 studies met the inclusion criteria and were included in the final analysis. The relatively limited evidence about the association between elements of ICU treatment and memories after ICU discharge suggest that deep sedation, corticoids and administration of glucose 50% due to hypoglycaemia contribute to the development of delusional memories and amnesia of ICU stay.ConclusionsThe body of literature on the relationship between elements of ICU treatment and memories after ICU discharge is small and at its early stages. Larger studies using rigorous study design are needed in order to evaluate the effects of different elements of ICU treatment on the development of memories of the ICU during recovery.

    Inom ramen för denna artikel diskuterar vi såväl den psykosociala som den fysiska miljön i skolan. Vidare argumenterar vi för att skolans miljö är av betydelse för elevernas möjligheter att lära sig, likväl som för deras upplevelser av lärandesituationerna. Särskilt intressant blir då att lyssna till eleverna. Syftet med denna artikel är därför att synliggöra och diskutera elevers röster i förhållande till hur skolan kan bli den bästa platsen för lärande. Detta görs med utgångspunkt i forskningsprojektet ”Skolan suger” eller?, där drygt 200 elever i åldrarna 11–15 år deltog. Eleverna fick skriftligen reflektera över hur skolan ska bli en riktigt bra lärandemiljö. För att utveckla skolan till den bästa platsen för lärande uttryckte eleverna en önskan att kunna påverka sin lärandemiljö, att uppnå ömsesidighet och samspel, att kunna hantera tidsregleringen i skolan, samt att uppfylla behov för välbefinnande.

    What can we do to improve our knowledge and understanding of school children health and well being? This question was raised when the ArctiChildren project started. The aim of this presentation is to share some methodological perspectives on research within schools. It is crucial to develop a number of different options of collaboration between researchers, teachers and students, as well as different possible methods. We will highlight the importance of developing collaboration between the researchers and the schools. We will also discuss projective methods, for example drawings, as one way to gain knowledge and understanding of schoolchildren's school experiences. Another method we will discuss is working on consulting basis with the staff in order to decrease bullying and harassment in school.

    Bakgrund: Omkring 90 000 personer avlider i Sverige varje år varav cirka 200–270 personer kan vara potentiella organdonationer. Det är betydelsefullt att intensivvårdssjuksköterskan känner sig väl förtrogen med rutiner kring identifiering av en potentiell donator och riktlinjer kring monitorering och behandlingsmål för organdonatorn. Syfte: Syftet med studien var att beskriva intensivvårdssjuksköterskors upplevelser av donationsprocessen utifrån två frågeställningar; vilka var de främjande respektive utmanande faktorerna i samband med donationsprocessen? Metod: En kvalitativ intervjustudie med ett ändamålsenligt urval genomfördes. Inklusionskriterierna var specialistsjuksköterska inom intensivvård med erfarenhet av donationsprocessen. En semistrukturerad intervjuguide användes och data analyserades enligt kvalitativ manifest innehållsanalys. Resultat: Sexkategorier framkom; Behov av utbildning och praktisk träning, att frågan om donation av någon anledning inte ställs, en sällan förekommande process som är resurskrävande och annorlunda, en positiv inställning och en uttalad vilja, betydelsen av stöd och information, intensivvårdsmiljö är rätt miljö. Slutsats: Studien visade att intensivvårdssjuksköterskor upplevde ett flertal utmanande och främjande faktorer under donationsprocessen. Det finns behov av fler studier inom området. En interventionsstudie före och efter specifik utbildning med både teoretisk och praktisk undervisning behöver utföras för att studera hur och i vilken grad utbildning påverkar donationsprocessen. Det finns en brist på studier som berör varför frågan om donation inte ställs och även detta behöver studeras vidare.

    Background: Breastfeeding problems are common during the early period but can often be prevented or overcome with adequate support. Child-health nurses meet almost all children during their first weeks of life and play an important role in promoting breastfeeding and in strengthening parents’ confidence and their belief in their own ability. It is, therefore, important to gain more knowledge about child-health nurses’ experiences. Objective: To describe child-health nurses’ experiences of providing breastfeeding support. Methods: This qualitative study is descriptive with an inductive approach. A purposive sample of eight child-health nurses recruited from district health care centers participated. Data were collected through focus group interviews and analyzed with content analysis. Results: Child-health nurses consider it to be important to provide early breastfeeding support and that early hospital discharge following birth can complicate breastfeeding. Furthermore, the introduction of infant formula and tiny tastes given to the baby can be a barrier to breastfeeding. Parents’ confidence had an effect on breastfeeding, and breastfeeding is promoted by confident parents. Trends and cultural differences have an influence on parents’ attitudes toward breastfeeding. Child-health nurses stated the importance of having a consensus breastfeeding policy. Conclusion and Recommendation: A number of factors affect breastfeeding, and breastfeeding support from child-health nurses is important in the early stages after birth. To conclude, the support must be individually tailored with a focus on the parents’ needs. There is a need for greater cooperation between the maternal care and child-health care staff in order to provide adequate and continuous breastfeeding support throughout the care chain.

    The aim of this study was to describe women's experience of living with a colostomy after rectal cancer surgery. Interviews with five women about their experiences were subjected to thematic content analysis. The findings showed that receiving a cancer diagnosis gave rise to thoughts about life and death. For the women to feel comfortable, the information and health-care measures need to focus on supporting them through the entire process, also when the treatment is completed. After the surgery, the women adjusted to living with colostomy and carried on as before the cancer diagnosis, but they constantly worried about leakage or flatulence. The women were happy to have survived the cancer and this realization helped them to accept and have a good life with colostomy. In conclusion, women with colostomy because of rectal surgery need specific rehabilitation and nursing care that focuses on adjustment to temporary or permanent changes in life.

    På senare år har strömmen av människor som flyr undan konflikt ökat markant. Det stora flertalet som söker sig till Sverige har en annan kulturell bakgrund med skilda värderingar och traditioner än den svenska. De har ofta ett modersmål som innebär svårigheter att kommunicera. En sviktande hälsa eller andra livshändelser gör att en stor del av dessa människor söker kontakt med den svenska sjukvården och då främst ambulanssjukvården eftersom de har fått information om det svenska nödnumret 112. Ambulanssjuksköterskors möten med människor som har en annan kulturell bakgrund eller där språket försvårar kommunikationen har blivit allt vanligare. Syftet med denna studie var att beskriva ambulanssjuksköterskors upplevelser av sitt prehospitala omhändertagande när språket eller den kulturella skillnaden är en begränsning. Nio personer intervjuades, alla var anställda som ambulanssjuksköterskor med erfarenhet att möta dessa patienter. Datainsamlingen utfördes med hjälp av semi-strukturerade intervjuer och analyserades med kvalitativ innehållsanalys. Analysen gav slutgiltigt två huvudkategorier: ”Språket som begränsning” och ”Kulturella begränsningar” samt sju kategorier. Resultatet visar att den kontext som ambulanssjuksköterskan verkar inom på många sätt är extrem och utsatt, där snabba beslut ibland måste fattas med hjälp av mycket bristfällig eller helt utebliven information och möjlighet till stöd. Svårigheter att kommunicera med patient och närstående tillsammans med kulturella skillnader skapar många gånger frustration hos de inblandade men det beskrevs även hur berikande dessa möten kan vara. Erfarenheter och därigenom nya sätt att hantera dessa situationer var talande för flertalet av deltagarna. En tydlig önskan om att öka sin kunskap inom detta område, att bli mer kulturellt kompetent, var återkommande. En slutsats som kan dras är att ytterligare kulturell utbildning för ambulanssjuksköterskorna samt tolkhjälp skulle bidra till en god kommunikation med patienterna. Ambulanssjuksköterskorna skulle ha större möjlighet att ge information på ett kulturellt anpassat sätt om de fick en ökad kulturell kompetens.

    he population is ageing the world over, and there is an increasing prevalence of chronic illness and complex conditions. Older people are at greater risk of having several complications than the general population, leading to more time spent in hospital and an increased risk of readmission. The most specific need of older patients is often the multiple need of care. The aim of this study was to describe older patients’ experiences of caretime during a hospitalization in a medical ward. Data were collected with semi-structured interviews with nine older patients, and analysed using qualitative thematic content analysis. The analysis resulted in two themes and five categories. The results show that caretime during hospitalization includes a lot of waiting and that patients manage the waiting in different ways. The results also point out the importance of patients developing good relationships with professionals since good relations creates feelings of security and can reduce anxiety and fear during a hospital stay. The patients pointed out the importance of being more involved in their own care and asked for more detailed information about the hospital stay. The results revealed that information makes patients’ more secure and safe during a hospitalization and makes them participate and affect their own care and treatment more.

    BACKGROUNDThe tension between care-based and technology-based rationalities motivates studies concerning how technology can be used in the care sector to support the relational foundation of care.OBJECTIVESThis study interprets values related to care and technologies connected to the practice of good care.RESEARCH DESIGNThis research study was part of a development project aimed at developing innovative work practices through information and communication technology. Participants and research context: All staff (n = 18) working at two wards in a care facility for older people were asked to participate in interviews, and 12 accepted. We analysed the data using latent content analysis in combination with normative analysis. Ethical considerations: The caregivers were informed that participation was voluntary and that they could drop out at any time without providing any explanation.FINDINGSFour values were identified: 'presence', 'appreciation', 'competence' and 'trust'. Caregivers wanted to focus on care receivers as unique persons, a view that they thought was compromised by time-consuming and beeping electronic devices. Appraising from next-of-kin and been seen as someone who can contribute together with knowledge to handle different situations were other desires. The caregivers also desired positive feedback from next-of-kin, as they wanted to be seen as professionals who have the knowledge and skills to handle difficult situations. In addition, the caregivers wanted their employer to trust them, and they wanted to work in a calm environment.DISCUSSIONCaregivers' desire for disturbance-free interactions, being valued for their skills and working in a trustful working environment were interpreted as their base for providing good care. The caregivers' arguments are based on caring rationality, and sometimes they felt the technological rationality interfered with their main mission, providing quality care.CONCLUSIONIntroducing new technology in caring should support the caring relationship. Although society's overall technology-based approach may have gained popularity as a problem solver, technology-based rationality may compromise a care-based rationality. A shift in attitudes towards care as a concept on all societal levels is needed.

    Abstract [en]

    This study was undertaken to describe staff expectations prior to implementation of new electronic applications in a changing organization. Changes are a part of human existence; changes based on implementation of technology and information and communication technology are taking place in the health care sector globally. The Swedish public health care and social care sector is taking a similar path. A qualitative approach with group interviews of 23 staff divided in 5 groups was performed. Latent content analysis was used to analyze the transcribed interviews. The theme, taking standpoint today in relation to the past, emerged from 3 areas of discussion: "distance holding," "ruled by the organization," and "health care development in the future." New restraints on staff affect the caregiving process. Managers should consider whether a particular change is revolutionary or evolutionary and act in the change process according to the possible psychological impact of the change.

    Digital support for medication administration: Status-enhancing innovation for care workers?2013Conference paper (Other academic)
    Abstract [en]

    Background: There are assumptions that innovative ICT solutions in healthcare can improve the efficiency and contribute to increased quality. Innovation through ICT is also expected to increase status of and attract men to care work.A project aimed at finding innovative ICT solutions was realized between 2009 and 2012. The project involved a healthcare center and two nursing homes. Technicians (all men) were those in the project given the assignment of finding innovative solutions together with staff. Involved in the project were also ICT companies and academic researchers. This research study was undertaken in a nursing home context. A tool for medication administration was under development and to be tested.Purpose; to describe staff’s perceptions of digital support for medication administration and understand staff’s underlying values when arguing for or against the tool.Data collection; focus-group interviews were carried out to collect data. To analyze and interpret the content of the data, a phenomenographic method was used.Findings; Participants questioned the utility and also the need of the ICT solution. Participants also expressed a risk for impaired working environment as an effect of the tool. They also thought the tool would be complicated to use and sometimes would lack in performance. Increased status as an effect of introducing ICT was a belief that was not held by the staff.Conclusions; Setting aside the fact that from the perspective of an outsider, the intention of the project was indeed to do good , staff did not perceive the ICT solution in a positive manner. Politicians and policymakers may be over emphasizing the possibilities of ICT for solving future challenges in healthcare. Focus on technological innovations and a discourse in which care workers are excluded can actually be contra-productive to the possibilities of enhancing the status of care work, which is still predominantly performed by women

    Bakgrund:Elektiva ortopediska operationer på grund av osteoartros är vanligt förekommande och antalet har ökat sedan protesoperationer började genomföras i början av 1960-talet. Vårdtiderna har förkortats och det ställer höga krav på god och säker omvårdnad. Patienter med osteoartros upplever ofta att de har en nedsatt livskvalitet och en operation kan ge förutsättningar till ett bättre liv. Syfte: Studiens syfte var att beskriva kvinnliga patienters upplevelse av att genomgå en ortopedisk operation. Datainsamling: Semistrukturerade intervjuer analyserades med en kvalitativ innehållsanalys som resulterade i tre kategorier och sexunderkategorier.Resultat: I resultatet framkom att information ses som viktigt både innan och under händelseförloppet, att det är viktigt att få ett bra bemötande för att upplevelsen ska bli trygg och att själva operationen kan ge känslor av både hoppfullhet och obehag. Hur sjukvårdspersonalens bemötande inverkar på patientens upplevelser av operation och en god vårdrelation har en stor betydelse för patientens upplevelse av sjukhusvistelsen och det egna välbefinnandet. Konklusion: När patienten upplever delaktighet i vårdförloppet av kompetent vårdpersonal känner de att omvårdnaden blir personlig och upplever därmed trygghet.

    Objectives: To test the reliability and validity of the Students' Attitudes Towards Addressing Sexual Health questionnaire (SA-SH), measuring students' attitudes towards addressing sexual health in their future professions.

    Method: A cross-sectional online survey (22 items) were distributed to 186 nursing, occupational therapy and physiotherapy students in Sweden, April 2015. Validity and reliability were tested.

    Results: The construct validity analysis led to three major factors: present feelings of comfortableness, future working environment and fear of negative influence on future patient relations. The construct validity, internal consistency reliability and intrarater reliability showed good results.Conclusion: The SA-SH is valid and reliable.

    The aim of this study was to explore differences and similarities in health care students’ attitudes towards working with and communicating with patients about sexual health issues in their future professions. The aim was also to explore whether the students’ gender, age and future professions were influencing factors and whether there was a change in attitude depending on educational levels, gender, age and future professions. The study also aimed to explore the potential development of those differences and similarities in attitudes between health care students having achieved different levels of education and training in their future professions. A cross-sectional quantitative study was performed with an online survey distributed to nursing, occupational therapy and physiotherapy students. The students believed that they needed increased sexual health education and increased communication skills about sexual health. Gender and future profession are factors that significantly affect the attitudes of the students towards working with sexual health. Nursing and occupational therapy students have a more positive attitude towards addressing sexual health in their future professions than do physiotherapy students. Further research is needed in this field to improve competence in sexual health for all student groups, particularly physiotherapy students. Further research is also needed to explore the significance of gender regarding education in sexual health and attitudes towards working with sexual health.

    Sexuell hälsa är en mänsklig rättighet och tillfredsställande sexualliv har stor betydelse för människors välbefinnande. Behovet av att tala om sexuell hälsa är olika beroende på patienten. Sjuksköterskan har en nyckelroll när det gäller kommunikation och ska se till patientens behov både fysiskt, psykiskt och andligt. Syftet med denna studie var att beskriva vårdpersonals upplevelser av samtal om sexuell hälsa med patienter. Genom en manifest kvalitativ innehållsanalys av elva vetenskapliga artiklar besvarades syftet och resultatet gav fyra kategorier: att kön, ålder, civilstånd och religion påverkar samtal, att arbetsmiljö försvårar möjligheten till samtal, att obekvämlighet och otydliga ansvarsroller påverkar samtal och att kunskap främjar trygghet. Resultatet visade på övergripande kunskapsbrist hos vårdpersonalen och med förbättrad utbildning och tydligare riktlinjer för vårdpersonalen kan detta förbättras. EX-PLISSIT modellen kan göra skillnad men även att verksamheterna måste ta del av forskningen för att komma ifrån problematiken med att samtala med patienter om sexuell hälsa.

    Bakgrund: Runt om i världen och på svenska sjukhus och , överrapporteras dagligen flertaletpostoperativa patienter mellan anestesi-och intensivvårdssjuksköterskor. Vad somkommunicerats har stor betydelse för patienternas fortsatta vård och sjuksköterskornasupplevelse av överrapportering. Bristande överrapportering kan få konsekvenser som sämrevårdkvalité och ökad risk för komplikationer för patienterna. Syfte: Att beskriva anestesi-ochintensivvårdssjuksköterskors upplevelser av överrapportering av postoperativa patienter. Metod:Intervjuer genomfördes i form av fokusgrupper, en med anestesisjuksköterskor och en medintensivvårdssjuksköterskor. Materialet analyserades enligt kvalitativ innehållsanalys. Resultat:Fem huvudkategorier: Behov av att dubbelkolla och dubbeldokumentera, att inte veta vilkeninformation som är relevant och sakna förståelse för varandra, att den postoperativaavdelningen är rörig och orsakar störningar i överrapportering, attoperationssjuksköterskornas överrapportering är viktig och behov av att överrapportera vidpatienterna och upprätthålla fullständig sekretess. Slutsats: Tydliga rutiner behövs gällande vadpostoperativ överrapportering ska innehålla, samt utveckling av samarbete mellan de som deltar iöverrapportering. Behov av att kunna genomföra överrapportering vid patienterna men ändåupprätthålla sekretess var ett komplext område. Operationssjuksköterskans deltagande iöverrapportering var ett av de största förbättringsområden som framkom denna studie.

    Abstrakt

    Att befinna sig i stadiet palliativ vård är något som påverkar hela individen, både kropp och själ. Sjuksköterskor har en betydande roll i den palliativa vården, för att kunna se och möta patientens behov på ett fysiskt och psykiskt plan. Syftet med denna litteraturstudie var att beskriva sjuksköterskors upplevelser av etiskt utmanande situationer inom palliativ vård. En systematisk litteratursökning genomfördes inom området och tio vetenskapliga artiklar valdes ut som svarade mot syftet. Artiklarna som ingick i analysen, analyserades enligt kvalitativ innehållsanalys med manifest ansats. Analysprocessen resulterade i fyra slutliga kategorier: Att det är svårt att leva upp till värderingar av palliativ vård, att brist på samsyn och stöd leder till maktlöshet, att inte ha kunskap att möta patienters och närståendes förväntningar skapar frustration och att känslor av otillräcklighet leder till osäkerhet vid patientmötet. Resultatet visade på en sammanställning av att sjuksköterskor kan möta palliativ vård i primärvård, slutenvård och hemsjukvård. Detta upplevde sjuksköterskor som en etiskt utmanande situation på grund av bristande kunskap om ämnet. Slutsatsen som framkom är att sjuksköterskor behöver en ökad kunskap samt mer utbildning om palliativ vård, för att kunna ge en värdig död. Det behövs dock vidare forskning inom etiken och dess påverkan av palliativ vård, för att kunna förbättra samt utveckla vården inom detta område.

    Abstract [en]

    Pain is common among elderly. Very sick elderly persons living in nursing homes often have communication problems because of dementia or stroke, which causes problem to judge pain and evaluate effects of pain treatment. The aim of this study was to describe the prevalence, localisation and expressions of pain and to evaluate the usefulness of some pain assessment methods for nursing home residents. The study included 66 elderly living in a nursing home of which 49% had dementia as main medical diagnosis. A specially trained registered nurse (RN) performed pain assessment by * presenting a set of pain assessment tools * interpreting the nursing home residents' reactions during activity and * interviewing the resident about perceived pain. Two physiotherapists (PTs) performed physical examination including assessment of joint motion and palpation of joints and muscles. Contractures and ambulating status were noted. Cognitive Performance Scale (CPS) assessed cognitive capacity. When assisted by the RN 61% of the residents used independent or by assistance a verbal rating scale. The RN made an estimation of pain in all the nursing home residents and the PTs examined 92% of the residents. Pain was identified in 88% of the residents, 42% of the residents were treated with analgesics and pain it-as documented in the nurses' daily notes in 27% of the patient records. All residents with contractures had pain in the engaged joint. This study indicates that it is possible to assess pain by combining interview and interpretation of the residents' reactions during activity. Detection of pain by physical examination is a valuable complement especially for persons with decreased ability to communicate.

    Upplevelser och erfarenheter av undersökningar i hemmet med hjälp av informations- och kommunikationsteknik beskrivet ur patienters och läkares perspektiv.Projektets övergripande syfte  Syftet med projektet är att belysa upplevelser och erfarenheter av undersökningar i hemmet med hjälp av informations. och kommunikationsteknik.    Kort beskrivning av projektet   I den första studien intervjuades 9 personer som fick vård i hemmet om deras syn på den nya teknik som distriktssköterskorna använde i hemsjukvården för diagnostik och kommunikation. I intervjuerna framkom stor tilltro till denna teknik men den kan inte överlåtas till patienterna själva eller deras anhöriga att sköta.I den andra studien gjordes gruppintervjuer omfattande totalt 17 distriktsläkare med varierande erfarenhet av en ny teknik som prövades i projektform. Uppfattningen om användbarheten av den nya tekniken vid intervjutillfället och i ett framtidsperspektiv varierade stort. Samtliga framhöll vikten av det personliga mötet med patienten som aldrig kan ersättas.I båda studierna användes kvalitativ innehållsanalys för att analysera intervjutexterna.

    Äldre personers föreställning om vård och användning av assisterande teknologi och service i händelse av vård   Projektets övergripande syfte  Syftet med studien var att beskriva äldre personers föreställningar om god vård och om användning av assisterande teknologi och service (ATS) i händelse av vård.    Kort beskrivning av projektet  Individuella intervjuer har genomförts med 12 par, där båda är > 70 år och utan någon form av regelbunden vård eller tillsyn i hemmet. Intervjuerna utgick från en tänkt situation (scenario) som komplicerades alltmer, där deltagarna ombads berätta om sin syn på god vård och användning av ATS både för egen del och även hur de trodde att partnerns inställning var. Intervjuerna var fokuserade till scenariot och utveckling av resonemang om god vård och användning av ATS vid vård och omsorg i hemmet. Intervjuerna spelades in på band, skrevs ut ordagrant och analyserades med kvalitativ innehållsanalys. Inför införandet av teknik vid vård i hemmet är det viktigt att beskriva hur äldre personer ser på användning av teknik och hur de ser på vad som är god vård.

    Introduction: To achieve quality care in the person's home implies that the way we perform the care has to be developed. In this development new technology services, including mobile distance-spanning technology and information- and communication technology, has to be used. The aim of this presentation is to explore some issues important to consider before implementation of e-health services in care of older persons at home. Method: The presentation is based on studies where healthy older persons (n=23), older persons who had experience of being cared for with mobile distance-spanning technology (n=9) and general practitioners (n=17) were interviewed about their perceptions, views and experiences concerning the use of technology in care at home and quality care. The interviews were analysed by qualitative content analysis. Results: In the results from the different studies there were some common results. One was the importance of human meetings; another was that the person in need of care and the health professional need to know each other if virtual meetings should be possible. Other issues brought up in the different studies were trust, quality care, the need of combining home care services with visits at health care centres and hospitals, different technical equipment are only useful in specific circumstances, and mobile distance-spanning technology should not be looked at separately but as one part of a complex chain of health care services where all parts of the chain are needed. Conclusion: E-health including mobile distance-spanning technology and information- and communication technology is on its way to be accepted and used in home care for older persons. It is important that nurses as professionals actively take part in the development, designing and testing of the technical devices.

    Abstract [sv]

    I och med att invandringen i Sverige har ökat så har behovet av tolkade samtal blivit större. Kommunikationen är grunden för en säker och fungerande vård. Syftet med denna studie var att beskriva distriktssköterskors upplevelser av tolksamtal inom primärvården.

    Resultatet i studien baseras på ostrukturerade intervjuer med nio distriktssköterskor. Intervjuerna analyserades med en kvalitativ innehållsanalys. Analysen resulterade i fem kategorier: Att planera inför samtalet, att skapa förutsättningar för ett lyckat samtal, att tolken är koncentrerad och objektiv, att rätt översättning är en förutsättning för trygg omvårdnad och att se fördelar med att välja en auktoriserad tolk. Resultatet visar att tolkade samtal tar längre tid och kräver mer planering. Att både distriktssköterskans och tolkens bemötande och förhållningssätt har betydelse för ett lyckat samtal. Distriktssköterskor känner osäkerhet att det de sagt tolkas rätt. De framhåller trygghet för patienten som en viktig del i samtalet och de föredrar auktoriserade tolkar framför kollegor och närstående till patienten. Sammanfattningsvis så visar resultatet av studien att det med tolksamtal både finns möjligheter och svårigheter där både distriktssköterskan och tolken ansvarar för att samtalet blir lyckat. Mer forskning behövs där tydliga rutiner med åtgärder och rekommendationer vid tolksamtal bör tas fram för att övervinna språkbarriärer i framtiden.

    Abstract [en]

    Background

    Attempts have been made to use dynamic spirometry to define restrictive lung function, but the definition of a restrictive spirometric pattern (RSP) varies between studies such as BOLD and NHANES. The aim of this study was to estimate the prevalence and risk factors of RSP among adults in northern Sweden based on different definitions.

    Methods

    In 2008–2009 a general population sample aged 21–86y within the obstructive lung disease in northern Sweden (OLIN) studies was examined by structured interview and spirometry, and 726 subjects participated (71% of invited). The prevalence of RSP was calculated according to three different definitions based on pre-as well as post-bronchodilator spirometry:

    1) FVC < 80% & FEV1/FVC > 0.7

    2) FVC < 80% & FEV1/FVC > LLN

    3) FVC < LLN & FEV1/FVC > LLN

    Results

    The three definitions yielded RSP prevalence estimates of 10.5%, 11.2% and 9.4% respectively, when based on pre-bronchodilator values. The prevalence was lower when based on post-bronchodilator values, i.e. 7.3%, 7.9% and 6.6%. According to definition 1 and 2, the RSP prevalence increased by age, but not according to definition 3. The overlap between the definitions was substantial. When corrected for confounding factors, manual work in industry and diabetes with obesity were independently associated with an increased risk for RSP regardless of definition.

    Conclusions

    The prevalence of RSP was 7–11%. The prevalence estimates differed more depending on the choice of pre- compared to post-bronchodilator values than on the choice of RSP definition. RSP was, regardless of definition, independently associated with manual work in industry and diabetes with obesity.

In Sweden smoking prevalence has steadily decreased over the past 30 years.     Abstract [en]

    Background and objective: Asthma is a major public health concern. The aim of this study was to characterize a large population-based cohort of adults with asthma, and to study factors associated with all-cause mortality and non-participation in a long-term follow-up. Design: Random and stratified samples from five population-based cohorts were clinically examined during 1986-2001, and all subjects with asthma were included in the study (n = 2055, age 19-72 years, 55% women). Independent associations between different risk factors and (i) mortality and (ii) non-participation in a clinical follow-up in 2012-2014 were estimated. Results: In 1986-2001, 95% reported any wheeze and/or attacks of shortness of breath in the past 12 months, and/or asthma medication use. Over the up to 28 years of follow-up time, the cumulative mortality was 22.7%. Male gender, current smoking, and lower forced expiratory volume in 1 sec of predicted (FEV1% of predicted) were independent risk factors for mortality, while obesity was associated with non-participation in the follow-up. Older ages, ischemic heart disease, and low socioeconomic status were associated with both mortality and non-participation. Conclusions: The risk factors associated with mortality in this adult population-based asthma cohort were similar to those commonly identified in general population samples, i.e. male gender, current smoking, and lower FEV1% of predicted, while obesity was associated with non-participation in a long-term follow-up. Ischemic heart disease, low socioeconomic status, and older ages were associated with both mortality and non-participation.

    Abstract [en]

    Background

    Asthma and COPD may overlap (ACO) but information about incidence and risk factors are lacking. This study aimed to estimate prevalence, incidence and risk factors of chronic airway obstruction (CAO) in a population-based adult asthma cohort.

    Methods

    During 1986–2001 a large population-based asthma cohort was identified (n = 2055, 19-72y). Subsamples have participated in clinical follow-ups during the subsequent years. The entire cohort was invited to a clinical follow-up including interview, spirometry, and blood sampling in 2012–2014 when n = 983 subjects performed adequate spirometry. CAO was defined as post-bronchodilator FEV1/FVC<0.7.

    Results

    At study entry, asthmatics with prevalent CAO (11.4%) reported more respiratory symptoms, asthma medication use, and ischemic heart disease than asthmatics without CAO (asthma only). Subjects who developed CAO during follow-up (17.6%; incidence rate of 16/1000/year) had a more rapid FEV1 decline and higher levels of neutrophils than asthma only. Smoking, older age and male sex were independently associated with increased risk for both prevalent and incident CAO, while obesity had a protective effect.

    Conclusions

    In this prospective adult asthma cohort, the majority did not develop CAO. Smoking, older age and male sex were risk factors for prevalent and incident CAO, similar to risk factors described for COPD in the general population.

    Background

    During the latter half of the 20th century, the prevalence of asthma and many other allergic diseases has increased. Information on asthma prevalence trends among adults after 2010, especially regarding studies separating allergic asthma from non-allergic asthma, is lacking.

    Objective

    The aim was to estimate prevalence trends of current asthma among adults, both allergic and non-allergic, from 1996 to 2016.

    Methods

    Three cross-sectional samples from the same area of Sweden, 20-69 years, participated in surveys with the same questionnaire in 1996 (n=7104 participants, 85% response rate), 2006 (n=6165, 77%) and 2016 (n=5466, 53%), respectively. Allergic rhino-conjunctivitis (ARC) was used as a marker for allergic sensitization to define allergic asthma.

    Results

    The prevalence of current asthma increased from 8.4% (95% CI: 7.8-9.0) in 1996 to 9.9% (95% CI: 9.2-10.6) in 2006 and 10.9% (95% CI: 10.1-11.7) in 2016 (P<.001). Allergic asthma increased from 5.0% (95% CI: 4.5-5.5) in 1996 to 6.0% (95% CI: 5.4-6.6) in 2006 and further to 7.3% (95% CI: 6.6-8.0) in 2016 (P<.001), while the prevalence of non-allergic asthma remained stable around 3.4%-3.8%. The increase in current asthma was most pronounced among women and among the middle-aged. Physician-diagnosed asthma, asthma medication use and ARC also increased significantly, while the prevalence of symptoms common in asthma such as wheeze and attacks of shortness of breath decreased slightly or was stable. The prevalence of current smoking decreased from 27.4% in 1996 to 12.3% in 2016.

    Conclusions and Clinical Relevance

    The prevalence of allergic asthma increased from 1996 to 2006 and further to 2016, while the prevalence of non-allergic asthma remained on a stable prevalence level. The prevalence of symptoms common in asthma decreased slightly or was stable despite a substantial decrease in the prevalence of current smoking. Clinicians should be aware that the previously observed increase in prevalence of allergic asthma is still ongoing.

    Abstract [en]

    Managerial documents for the national school system in Sweden have emphasised taking students’ voices as a starting point in forming education, and several previous studies have indicated the benefits of giving students opportunities to participate in school. This study aimed to explore students’ reflections on what they would do if they were to decide how to make school the best place for learning. A total of 200 students aged 11 to 15 years from four schools (rural and urban) in two municipalities in the northern part of Sweden participated. The empirical data consisted of the students’ written reflections. The findings fall within four themes: (i) influencing educational settings; (ii) striving for reciprocity; (iii) managing time struggles; and (iv) satisfying well-being needs. Tensions between the students’ previous experiences and future visions appeared. The findings can offer direction regarding aspects of the learning environment in school that could be improved.

    Abstract [en]

    The aim of this study was to describe, reflect upon, and create a deeper understanding of aspects relevant for promoting a positive school environment from a student perspective. The data was analyzed by using an inductive phenomenological method and based on written responses from 200 Swedish students from grades 5–9. The results indicated that the students found aspects within, as well as beyond, the classroom relevant for a positive school environment. For instance, outings were considered relevant for building and maintaining friendships and for learning processes. Moreover, the students discussed formal and informal conditions and considered social as well as structural circumstances important for having a good time in school. The relation between learning and well-being was also emphasized by the students.

    Background: Hypothermia is one of the most common complications of anesthesia. The consequences of hypothermia include disorders of coagulation with increased risk of bleeding, wound infections and cardiac complications. Patients have described hypothermia as the worst experience during hospitalization and value it higher than the surgical pain. Aim: The aim of this study was to describe the anesthesia nurse's experience in preventing hypothermia associated with anesthesia. Method: The study has a qualitative approach. Ten anesthesia nurses, active in an operations department in Västra Götaland county and the region of Västernorrland, participated in the study. The hospitals included in the study were similar in size and what type of surgery performed there. Qualitative semi- structured interviews were used to gather data and the interviews were analyzed with a qualitative content analysis. Results: The analysis resulted in three categories and shows that prevention of hypothermia is important according to the anesthesia nurses and that awareness about hypothermia and its prevention has increased. At the same time, it was described that more could be done. The anesthetic nurses felt that there are good practices that increase safety in the prevention of hypothermia. However, it was described as difficult to keep themselves up to date of the latest research. In order to increase the prerequisites and to further develop the staff group, in hypothermia prevention, a person responsible for hypothermia prevention is needed. Conclusion: Knowledge, experience, research and collaboration in the operations team are fundamental factors for preventing hypothermia in the patient, in the best possible way. The anesthetic nurses do not experience hypothermia as a daily problem but mean that more could be done, within that area. More research is needed regarding the usefulness of a person responsible for hypothermia prevention would represent.

    Schizofreni beskrivs som ett svårt psykiatriskt tillstånd och en komplex gåta. Det är inte en vanlig sjukdom men den utgör en av de mest betungande och kostsammaste i världen. Sjukdomen medför en ofrivillig isolering, ett outhärdlig lidande samt osäkerhet till livet i stort. Sjuksköterskor har en central roll i mötet med dessa patienter och för dess tillfrisknande. För detta krävs det att man förstår samt tar del av patientens unika livsvärld. Syftet med denna litteraturstudie var att beskriva upplevelsen av att leva med schizofreni. Den systematiska litteratursökningen resulteradei 15 stycken vetenskapliga artiklar med kvalitativ ansats. Dessa analyserades genomen kvalitativ innehållsanalys med fokus på det manifesta. Fem stycken slutkategorier kunde slutligen presenteras: att uppleva en förändrad verklighet, att uppleva maktlöshet och sociala utmaningar, att uppleva trygghet i sjukvården, att känna sig älskad och finna gemenskap samt att uppleva hopp och finna copingstrategier. Resultat visade att personer med schizofreni upplevde ensamhet till följd av förlorad självkontroll samt stigmatisering. Den förlorade kontrollen över tillvaron resulterade även i en känsla av maktlöshet. Slutsatser av denna litteraturstudie var att ökad kunskap gällande sjukdomen har en positiv inverkan på individenslivskvalité. Kunskap avser så väl personens egen kunskap samt omgivningens förståelse för hur denna sjukdom ter sig.
