    Abstract [en]

    The aim of this study was to explore the phenomenon of physiotherapy when fathering and mothering a child with impairment. Parents of 22 young adults aged between 15 and 25 years were interviewed. A hermeneutic phenomenological analysis was carried out in several steps by alternating between individual analysis and reflection, collaborative group analysis and continued reflection. The meaning of physiotherapy could be understood by three relationships: (1) the fathers' and mothers' experiences of their relationship to the child, (2) their relationship to the physiotherapists and (3) their relationship to the physiotherapy practice. Within each relationship a variety of aspects emerged. The parents perceived themselves as needed, and perceived that the physiotherapy services did not meet the parents' needs. Fathers and mothers experienced the lack of support in different ways and physiotherapy was taken for granted by almost all parents.

    Abstract [en]

    Although palpation is frequently used in physiotherapy examinations, a limited amount of research has been done to clarify this concept. The aim of this study was to describe physiotherapists’ experiences of shoulder palpation with the intention of obtaining detailed descriptions of the concept of palpation. Three focus group interviews were conducted, including a total of seven physiotherapists who had experience of patients with shoulder disability built up over a long period. A qualitative content analysis of the interviews was carried out. The findings showed how palpation can be perceived as a skill that could be described from two different perspectives: Manual and technical palpation and Palpation being dependent on the relationship between the physiotherapist and the patient. Palpation requires time, and the knowledge and experience of palpating built up over time. We also found how the two “perspectives” of palpation were inseparable from each other. The ability to perform manual and technical palpation and the relationship between the physiotherapist and the patient can be considered an interactive learning situation where the physiotherapist and the patient develop mutual knowledge of the body.

    Abstract [en]

    There is a need for a deeper understanding of and discussion about different cultures' influences on life-views and ethical reasoning among professionals within healthcare. A culture can be seen as a set of guidelines, both explicit and implicit, which individuals inherit as members of a particular society, and which tells them how to view the world and how to behave in it in relation to other people. Do the cultural differences between Sweden, a culture predominantly influenced by Christian religion, and Turkey, a culture predominantly influenced by Muslim religion, influence physiotherapy students' life-views and ethical reasoning? The aim of this study was to compare life-views and questions of ethics between Swedish and Turkish physiotherapy students at the beginning of their studies. A reliable and valid questionnaire about life-views, together with questions covering ethical aspects was used. Three hundred and thirty-two physiotherapy students (186 Swedish, 146 Turkish) answered the same life-view questionnaire. Non-parametric statistics were used. Significant differences between the Swedish and Turkish students' opinions were analyzed with non-parametric statistics (Mann-Whitney U-test, two independent samples). Students' priorities in ethical questions were also compared between the groups. The Turkish students had significantly higher mean values in questions that belonged to a scientific life-view; the Swedish students had significantly higher mean values in questions belonging to an evolutionary life-view. There were significant differences between the groups in 14/20 individual life-view questions. The Swedish students were more patient-centered in their attitudes, had more positive attitudes to unconventional lifestyles and accepted to a higher extent than Turkish students suicide and euthanasia in special situations. Concerning priorities in ethical questions, there were significant differences between the groups concerning justice in healthcare and happiness for all, with higher priorities for happiness and justice in healthcare from the Turkish students.

    The aim of the study was to investigate what factors the physiotherapy experts in psychiatric physiotherapy believed to be important in the interaction between the patient and the physiotherapist (PT). The objective was to obtain a deeper insight into their reasoning concerning interaction and treatment outcomes. Eleven nominated 'expert' PTs were each interviewed twice. Triangulation of important events, an exemplar (critical incident) and a key informant interview were used in the data collection. A qualitative design with crosscase analysis was used in the data analysis. Important interaction factors were divided into a 'prerequisites dimension', 'interaction dimension' and 'outcome dimension'. In the prerequisites dimension there were three themes: 'prerequisites of the PT', 'prerequisites of the patient' and 'external factors'. In the interaction dimension there were also three themes: 'ways of contact', 'therapeutic process' and 'structure of treatment'. In the outcome dimension the theme 'outcome' and the two categories 'patient - awareness of own resources and 'PT - improvement of interaction skills with the aid of reflection' were found. Interaction was considered to be important for the outcome and for the patient's awareness of his/her resources. The body awareness skills of the PT, the therapeutic relationships, help for the patient to identify his/her resources and the contract were also important. The ability to be fully mentally present in the encounter and the use of non-verbal skills were emphasized. Supervision and reflection about the outcome increased the PT's interaction skills in a learning process. The physiotherapy process expressed was resource-oriented.

    Low back pain (LBP) is a common disorder in the western world. Persistent LBP can be caused by pathological changes in the discs and disturbed neuromuscular activation, which can cause hypotrophy of the strong type 2 muscle fibers of the extensors. Deadlift (DL) is an exercise that may address all these pathological transformations. The aim of the study was to investigate the effect of DL training on patients with persistent LBP. A single subject experimental design with an AB-design and multiple baselines was applied in this pilot study. Two patients with discogenic LBP and one with arthrogenic LBP were treated with DL training over 8–10 weeks. A follow-up was performed at 15 months. The subjects with discogenic LBP showed positive response to DL training regarding pain intensity and functional status, but the exercise did not affect their mental health. The subject with arthrogenic LBP did not seem to have positive effect from DL training. As a result of this pilot study, the authors hypothesize that DL training may be a successful treatment for subjects with LBP of discogenic origin.

    Abstract [en]

    The aim of the study was to evaluate a high intensive exercise program in stroke subjects with risk of falls regarding balance, activities of daily life, falls efficacy, number of falls and lifestyle activities. The intervention program contained high intensity functional exercises (HIFE) implemented to real-life situations together with education on falls and security aspects. This was a single-center, single-blinded, randomized controlled trial. Consecutive >55-year-old patients with risk of falls were enrolled and randomized 36 months after first or recurrent stroke to the intervention group (IG, n=15) or to the control group (CG, n=19) who received group discussions about hidden dysfunctions after stroke. Outcomes were Berg Balance Scale (BBS) primarily, Barthel Index (BI), Falls Efficacy Scale International (FES-I) and number of falls secondarily and Frenchay Activities Index last 3 months (FAI-3) tertially. Assessments were done at baseline, post-intervention, 3- and 6-month follow-up by two physiotherapists and one nurse blinded to group allocation. Generalized Estimating Equations with Repeated-measure statistics were used to analyze the data. There were no significant differences between the IG and the CG regarding balance (BBS). BI at 6 months and FES-I post-intervention and 3 months follow-up showed significant improvement in the IG compared with the CG (p<0.05). Number of falls and FAI-3 were without significant change. This study suggests that our program consisting of HIFE implemented in real-life situations together with educational discussions may improve performance of everyday life activities and improve falls efficacy in stroke subjects with risk of falls

    In this 1-year prospective study, the aim was to investigate if there are any differences concerning injuries and injury incidence during an entire football season between Swedish female football players from different geographical regions. A total of 130 out of 253 players (51%) sustained 229 injuries. The overall injury incidence was 9.6/1000 h of football. The incidence of injury during practice was 8.4/1000 h and during game was 13.3/1000 h. Players in the north had a higher incidence of injury during game than those in the south (19.5 vs. 7.2/1000 h, respectively, p<0.001). Fifty-nine per cent of all injuries were traumatic injuries. Both traumatic and overuse injuries occurred mainly during the early preseason and at the beginning of the competitive spring season. Of all injuries, 13% were classified as slight, 39% as minor, 37% as moderate and 11% as major injuries. Players in the north had a higher injury incidence concerning moderate injuries than players in the south (p<0.01). In the present study, evidence is presented that regional factors play a role in the injury incidence. Female football players in the north and south have different injury patterns. This could be a result of different conditions for football as a sport between the regions.

    We investigated whether the use of dynamic ankle foot orthoses (DAFOs) can improve the coordination between standing posture and a reaching movement. We investigated the temporal sequence between onset of center of pressure (CoP) displacement and onset of hand displacement by means of two AMTI force plates and a two-camera optoelectronic system (ELITE). The task was to reach for a cup. Reflective markers were placed on hand, trunk and the cup. Four children with spastic diplegia and eight control children (aged 5-12) participated. They were tested wearing DAFOs (diplegia) and the results were compared with wearing shoes (diplegia and control). The children with spastic diplegia were able to make use of anticipatory changes in CoP displacement. The results indicated that more trials in children with spastic diplegia had CoP onset preceding hand onset while wearing DAFOs (18/20) than when wearing only shoes. More trials (27/40) during the 20% reaching condition in the control group had CoP onset preceding hand onset than during the 10% reaching condition (18/40). In spite of different amounts of assistance, anticipatory postural adjustments were found in the diplegia group. Use of anticipatory postural adjustments was not consistent in the control group. Practice of reaching during standing can be carried out with DAFOs and/or hand support, and can promote motor learning of anticipatory postural adjustments.

    The aim of the study was to describe, using qualitative phenomenographic interviews, how physiotherapists experience client participation in physiotherapy interventions. The objective of phenomenographic research is to identify and describe various ways of experiencing the investigated phenomenon. Eleven respondents were strategically selected according to the maximum variation strategy with variations in: gender, seniority and areas of physiotherapy. Three qualitatively different descriptive categories were indentified with critical variations in paradigms of health and goal-setting procedures - A: Collaboration, i.e. an experience of client participation from a biopsychosocial collaborative view of intervention in which the client enter into equal partnership with the physiotherapist and thereby are jointly responsible for intervention, goal-setting and outcome; B: Guidance, i.e. an experience of client participation from a guided biomedical view of intervention, in which the client is guided by the physiotherapist in an unequal partnership in intervention and goal-setting; and C: Expertise, i.e. an experience of client participation from a paternalistic, biomedical view of intervention, in which the client sees the physiotherapist as an expert, who decides and controls the intervention and goal-setting. The results have made some earlier tacit professional physiotherapy knowledge explicit and may increase the understanding of how different experiences of client participation influence interventions

    The aim was to investigate the possibility of increasing lower limb strength, dynamic balance and gait speed in frail older women with residual mobility problems following hip fracture by using a combined and functional training programme with a weighted belt. Three women aged 78-82 participated in this repeated single-subject experimental design study. The intervention phase consisted of a 10-week functional weight-bearing exercise programme performed twice a week. Main outcome measures were assessed twice a week during baseline and intervention phases. Graphic analyses showed substantial improvements in dynamic balance and gait speed, related to the intervention, in all three subjects. During intervention phase, gait speed improved by 16-38% and step test performance (dynamic balance) by 36-67%. The subjects also increased weight load in the belt by 25-80%. No progress was seen regarding isometric knee extension muscle strength, and the control variable, isometric hand-grip strength, remained unchanged. However, dynamic leg strength (one-repetition maximum in leg press, tested before and after intervention) showed substantial increase. Subjects experienced strong improvement and reported no discomfort. In conclusion, functional training with a weighted belt may be suitable for older women to alleviated mobility problems following hip fracture.

    Abstract [en]

    Within healthcare, there is a growing interest in patient influence and participation. The aim of this study is to describe patients' experience of participation in and influence on rehabilitation with the focus on physiotherapy. Interviews with patients from three different primary healthcare centres were conducted. The result indicated low patient participation in and influence on the rehabilitation process within the hospital. The informants trusted the competence of the caregivers and tried to be as compliant as possible. Their experiences were that training must be supported and followed up. Information about rights was requested. The informants felt that they were listened to and confirmed by the physiotherapists. The positive attitude was combined with low demands and a great feeling of gratitude as a matter of course. Using information and support from the physiotherapist, together with friends and next of kin, the informants had learned to cope with the new situation. In this paper, these results are understood and described in terms of the parallel process of rehabilitation, based on traditional and individual models, and implying that the patient copes with the situation by shifting between being compliant and adopting more self-regulatory behaviour, for example. Attitudes are changing in society and this study reflects the patient of today, and is confirmed by recent studies

    Abstract [en]

    The aim of this study was to describe experiences of acupuncture treatment among women with migraine. The study was based on narrative interviews with 10 women in the northern part of Sweden. The interview text was analysed using qualitative content analysis and the results were grouped into two categories: "experiences during acupuncture treatment" and "experiences after acupuncture treatment" and five subheadings. The experience of acupuncture during treatment varied between women, over time, and depending on external circumstances. Women stated that the patient-therapist relationship affected the experience both during and the outcome after acupuncture treatment. The results showed that acupuncture relieved pain, decreased the use of pharmaceuticals and increased emotional strength. When the migraine disappeared or was lessened, women felt that they could live life to the fullest again and felt that family and work did not suffer, as they did before the acupuncture treatment. Women felt a feeling of control over migraine and experienced safety in the effect of acupuncture. In conclusion, acupuncture seems to relieve the consequences of migraine and can be viewed as an alternative for physiotherapists in treating persons with migraine.
